When you contact us, you will be in touch with the Center's coordinator who will walk you through the process step-by-step and help you to understand every aspect of your care. You can print and bring the completed forms with you to your first appointment or fax or mail them to us prior to your first appointment.
If you fax or mail your forms prior to your appointment, please retain copies for your records. You may also want to bring these copies with you to your first appointment.
