Release of Information Department is dedicated to processing your requests for patient-protected health information in a timely manner, while maintaining patient confidentiality as our top priority. Our hours of operation are Monday through Friday, 8:00 a.m. to 4:30 p.m.

For your convenience, we have one mailing address for all Memorial Hermann Hospitals and Outpatient Centers. To request your medical record, please complete the authorization form and mail back to:

Frequently asked questions

Can I send records to my physician?

If you need your records for medical care and would like for us to fax records to your physician, you may fax the completed authorization form to our office for processing. Facility information is listed below. Please make sure to include the doctor's phone number, as we must confirm their number before faxing your medical records.

To check status of your request, you must call the facility where the patient was seen. Release of Information Department customer service numbers are listed below.

Is there a fee for copies of medical records?

Yes, there is a fee for copies of medical records. The fees are set by the Texas Health and Safety Code 241.154(e) and updated annually to reflect the most recent changes to the consumer price index as published by the Bureau of Labor Statistics.
  1. A basic retrieval or processing fee that must include the first 10 pages of copies not to exceed $44.35 for pages 1-10
    1. $1.49 for each of pages 11-60
    2. .74 cents for each page of pages 61st-400th
    3. .39 cents for each page of any remaining pages of 401st and up
    4. The actual cost of mailing, shipping or delivering copies of records
  2. If the records are provided on a digital or electronic medium, a basic retrieval or processing fee, which may not exceed:
    1. $80.36 for CD
    2. The actual cost of mailing, shipping or delivering the provided copies
  3. If the requested records are stored in microfilm form, a basic retrieval or processing fee that must include the first 10 pages of copies not to exceed $67.57 for pages 1st-10th and:
    1. $1.49 for each page thereafter, and
    2. The actual cost of mailing, shipping or delivering the provided copies

The fees are set by the Texas Health and Safety Code 241.154 (e) and updated annually to reflect the most recent changes to the consumer price index as published by the Bureau of Labor Statistics.

Is there a fee for just viewing my medical records?

There is no fee for viewing your own medical records.  An authorization form must be completed and sent to Release of Information Department.  Because space is limited, appointments are scheduled for a maximum of 2 hours at a time.  Please call the hospital Release of Information Department to make an appointment.

Who owns my medical records?

Medical records are the hospital's documentation of a patient's medical care and services provided.  They are the property of MHHS.

What if my medical records information is inaccurate? Can I request an amendment to my medical records?

    If the change is for misspelling of your name or insurance information, please call (713) 448-5502.

    For other changes to the medical record, please download and complete the Amendment Form  and follow the instructions below:

    1. Written requests for amendments to medical records are to be mailed to:

    1. Memorial Hermann will contact the individual who wrote the information that is being requested to be amended.
      1.  The author of the note can agree with the request and the information will be amended;
      2. The author of the note can reject the request and the information will remain as originally documented.
    2. Memorial Hermann will respond in writing within 60 days of receipt of the request for an amendment.

    What number do I call to check status of my request?

      Patient Rights  

      1. You have a right to request access to and inspection of your medical record.
      2. You have a right to request a readable hard copy of your medical record.
      3. You have a right to request an amendment to your medical record.
      4. You have a right to a timely response to your request.
      5. You have a right to pick-up your requested medical record hard copy in person or ask that it is mailed to you.
      6. If your request to access, inspect, or obtain a hard copy of your medical record is denied, you have a right to have the denial decision reviewed by a licensed professional that was not directly involved in the denial decision.
      7. You have a right to request a summary of the medical record or an explanation of the summary.
      8. You have a right to know the cost of a hard copy, summary or explanation of the summary in advance.
      9.
